Back In Form is owned and operated by Physical Therapists who have 50 years of combined experience in the physical therapy field. Our physical therapist owned facility, run from the top down by clinicians, ensures that patients are always at the core of every decision we make. When you become a new patient you’ll be paired with the PT best-aligned with your unique needs and goals. Together, you’ll develop a personalized plan of care that will serve as the foundation of your care moving forward. Physical Therapy is designed to address functional limitations and improve one's quality of life through manual therapies, modalities and exercise programs.
LSVT BIG
Transform your mobility and independence with our LSVT BIG program, designed specifically for individuals with Parkinson’s disease. This evidence-based therapy focuses on increasing the amplitude of movements and enhancing balance, strength, and coordination for everyday tasks like walking, reaching for items in your closet or kitchen, or standing up from a chair. Through personalized one-on-one sessions with certified therapists, you’ll relearn how to make purposeful, larger motions to regain control over your symptoms and improve your quality of life.
Recovery Sessions
Return to activities faster with our Recovery Session, designed to enhance muscle recovery, reduce soreness, and improve flexibility. This 30-minute session incorporates cutting-edge modalities such as the Hypervolt massage gun, CBD-infused massage cream, the Venom Heat and Massage Belt, and Normatec Compression to target lactic acid buildup, optimize mobility, and alleviate pain. Perfect for athletes or anyone seeking improved physical performance and faster recovery.
Pre-hab Programs
Prepare your body and mind for surgery with our Pre-hab Programs. These programs focus on enhancing strength, flexibility, balance, and neuromuscular coordination to optimize recovery and reduce post-operative complications. By setting achievable goals and building confidence through guided physical activity, pre-hab improves overall surgical outcomes. Starting 1-2 months before surgery, our tailored programs help you feel ready, recover faster, and maintain long-term wellness.
Posture Restoration
Improve your posture and relieve discomfort with our Posture Restoration program. Designed by physical therapists, this program addresses common postural issues caused by prolonged sitting and sedentary habits, such as neck, back, and shoulder pain. Through movement analysis, personalized exercises, and education, we help you restore alignment, alleviate pain, and enhance your overall quality of life. You’ll also receive a home exercise plan to maintain these improvements for the long term.
Vestibular Therapy
We offer vestibular rehabilitation, a specialized form of physical therapy aimed to alleviate symptoms of vertigo and other vestibular disorders. Symptoms can include dizziness, spinning, lightheadedness, imbalances, and fear of falling. Our treatments are based on a comprehensive musculoskeletal, balance, gait and vertigo assessment to develop a customized plan to help you get back in form!
Golf Fitness Studio
Access our state-of-the-art golf fitness studio with a program designed by Michael Hauser. Our custom evaluation utilizes Titleist Performance Institute Program and Indoor Simulator with Flightscope Technology. Learn how to use your body efficiently, hit the ball farther and prevent injuries. One visit evaluations begin at $250, with packages up to 6 weeks with Michael Hauser available. Orthotics
Orthotics improve foot biomechanics which allows your body to move better. This can help with knee, hip, and back pain by restoring neutral biomechanics when standing and walking. Everyone can benefit from having good support from the ground up. Custom orthotics available at $250 / pair.
Theragun & Rock Tape

Massage Therapy
We offer Deep Tissue, Swedish and the use of Active Release Techniques from an ART Certified massage therapist. Enjoy the combination of relaxation and healing.
